Service access forms - Imaging platform

For services subject to a user fee, the relevant forms are available for consultation and download in this section

The competent Contact Person will provide the user with a detailed estimate of the costs calculated based on the agreed service. This estimate must be signed for acceptance by the client commissioning the work and will serve as a technical annex to the subsequent contract.

Upon acceptance of the estimate, the client will enter into a regular contract for third-party services with the Administrative Unit of the Imaging Platform. Based on this contract, a regular invoice will be issued upon delivery of the agreed work.
